Parent's Checklist
1. Completed Baptismal Registration Form
2. Permission to Baptize (Only if registered at another parish)
3. Child's birth certificate
For anyone who is to stand as Catholic Godparent, he/she must have received the sacraments of Baptism, First Communion and Confirmation. If married or living in, he/she must be married in the Catholic Church. Without these sacraments, he/she may not stand as Godparents/sponsor. (Canon Law 874)
4. Godfather:
a) Verification copy of reception of Sacramento of Confirmation
b) Catholic Marriage certificate (if married)
5. Godmother:
a) Verification copy of reception of Sacramento of Confirmation
b) Catholic Marriage certificate (if married)
6. Church donation (cash or check): $50.00
7. Appointment with St. Elizabeth Parish Pastor Rev. Jean Paul Khoury

8. Attend a Pre-Baptism Class (parents and godparents) - Date: _____

Note: if you are sending a picture of the filled-out form on paper, make sure the information is readable both in paper and in the picture.
